This issue happens when you delete the default contact form, and created a new one with a different name and id.
This causes the contact page template to search an unknown contact form.
If you create a new contact form and you want it to appear in the contact page template, please modify the included shortcode from page_contact.php: http://screencast.com/t/i1h1p779f3a
